Home Care Packages

Packages designed to give people the help they need to continue living in their own homes.

Home Care Packages are allocated by the Australian Government following an assessment by A.C.A.T. (Aged Care Assessment Team)
The package level approved will depend on the level of need.

Parkside is able to support all levels (1 - 4) of packages.  We can help you to design your personal support package.

Where more complex care is required, Parkside is in partnership with Watson Blanch Care Consultants. They can provide a comprehensive assessment of your need so you can make informed choices about how we can organise the best support for you.

All Home Care Packages are operated on a Consumer Directed Care (CDC) basis.

Eligibility

Referrals are made through MyAgedCare.  To receive a package you must have an assessment and be approved for a packaged by the Aged Care Assessment Team (ACAT).  Once approved you will be placed on the National Prioritisation List.
