Accessing Services

In order to obtain services from Parkside, you will need to make an initial contact with one of the Service Managers.  This is a process we call referral.  A referral can be made by an individual, a family member, advocate or service provider, but the person making the referral must have the consent of the person who is being referred.

Once a referral has been received, one of the Service Managers or Coordinators will make an appointment to conduct an assessment.  The assessment will help us determine if you are eligible for services and involves completing a document we call the 'Consumer Plan'.  The person conducting the assessment will also conduct an Occupational Health and Safety Assessment with your consent.

Once the assessment has been conducted, you will be prioritised for service delivery.  In some cases we can provide support immediately to eligible clients or carers, but unfortunately sometimes we have to place people on a waiting list.
