The Home Care Packages are designed to give people the help they need to stay living in their own homes. Packages can provide a range of services at times to suit the individual.
We provide HCPs in the Clarence, Hobart and Glenorchy areas. Priority is given to people from culturally and linguistically diverse (CALD) backgrounds, veterans, Aboriginal & Torres Strait Islanders (ATSI) and the financially disadvantaged.
Packages are individually planned and tailored to help older Australians remain living in their own homes. They are very flexible and designed to help with individual care needs. The types of services that may be provided as part of a package include assistance with personal care, cleaning, gardening and transport for shopping, or to attend appointments or social activities.
Once a client is offered a Package, staff meet with them and assist them to discuss the services they want and who they would prefer to carry out those services. Parkside staff keep in regular contact with clients, ensuring that services are delivered as the client wishes. Clients only have to contact us, no matter how many types of support they receive, so it's far less confusing for everyone. We will do all the organising and contacting of people.
The types of services that may be provided as part of aHome Care Package include help with
- meal preparation
- temporary in home respite, and
- social activities.
Example of a Home Care Package
(Mrs Smith is a fictitious character, and does not represent any individuals supported by Parkside.)
Mrs Smith has been allocated five hours a week of support. Mrs Smith contacts the Parkside Foundation in advance and requests support with tasks or recreation activities at a time that is suitable for her.
Mrs Smith usually allocates her hours to the following:
- 2 hours of light cleaning and household duties, spread throughout the week, generally on Monday and Friday.
- 1½ hours for grocery shopping, usually on Wednesday afternoon.
- 1½ hours of gardening, including watering and mowing, usually on Tuesday afternoon.
Although this is generally how Mrs Smith divides her hours, the hours are completely flexible and can be changed from one week to the next to suit her lifestyle.
This service is for older people with complex care needs.
In order to be eligible for a Package, a client must have been assessed by the Aged Care Assessment Team (ACAT). ACAT puts together a wait list of all people waiting for a package and that list is sent to all providers of Home Care Packages.
